While clonazepam effectively suppresses generalized EEG abnormalities, its benefits extend to focal epilepsies as well. The absorption of clonazepam from the gastrointestinal tract is efficient. In adults administered a 2 mg oral dose of micronized clonazepam, peak blood concentrations of 6.5-13.5 ng/mL were observed 1 to 2 hours after ingestion. However, in some individuals, maximum concentration occurs between 4 to 8 hours. Therapeutic effects in plasma concentrations are believed to lie between 20-80 ng/ml.
Sustained therapeutic levels can be achieved in adults with a daily dosage of clonazepam divided into three doses, while children aged 6 to 13 years are recommended 1.5-4 mg daily in three doses. The onset of action is typically within 20 to 60 minutes, lasting 6 to 8 hours in younger individuals and exceeding 12 hours in adults.
The distribution of clonazepam is analogous to other benzodiazepines, although there is limited information available. It is known to cross the blood-brain barrier and placenta. Clonazepam undergoes hepatic metabolism to generate several metabolites, including 7-aminoclonazepam, 7-acetaminoclonazepam, 3-hydroxy derivatives, and the parent compound. These metabolites are excreted mainly in urine, along with their conjugates.
