Mechanism of action:
Codeine. Opioids interact with stereospecific and saturable sites in the CNS and other tissues, with high concentrations in the limbic system, thalamus, stratum, hypothalamus, midbrain, and spinal cord. (Snayder et al 1976; Simon and Hiller 1978 Gilman et al 1990).
Their effect may result from mimicking the actions of enkephalins, beta-endorphins, and others that have been found to occupy the same binding sites in the CNS.
These actions appear to involve the degree of neurotransmitter release. In the nervous system, opioids and exogenous peptides inhibit the release of acetylcholine, norepinephrine, and substance P, and alter the release of dopamine. (Snyder 1978, Terenius 1978).
The affinity of codeine for opioid receptors is extremely low.
The analgesic effect of codeine is due to its conversion to morphine.
The antitussive action seems to involve specific receptors that bind codeine itself (Gilman et al. 1990).
Although codeine has been shown to reduce the frequency of coughing fits, and these effects seem to have a dose-response relationship, codeine does not alter respiratory vital capacity or the consistency or volume of sputum.
Bromhexine was studied using compounds labeled with C14, showing that immediately after oral absorption and after passing through the liver, it acts as a proteolytic enzyme, working on the molecular structure of sputum, breaking its bonds, giving rise to subunits of the structure. An activation of sialyltransferase occurs, with the consequent increase in sialomucins in bronchial and pulmonary mucus, correcting the amount of mucus secreted to normal. It also stimulates the rate of immunoglobulin A. In this way, the viscosity and elasticity of the mucus is regulated, facilitating its transport by the cilia.
