The pharmacological actions of benzodiazepines depend on the activation of 2 specific receptors. The central benzodiazepine receptor, present in various areas of the Central Nervous System (CNS), which is a component of the more complex structure that comprises receptors called GABA-A (gamma-aminobutyric acid), stimulation of these structures increases neurotransmission GABAergic, with intense inhibitory and neuroendocrine effects, and the peripheral benzodiazepine receptor, functionally and structurally different from the GABA-A receptor, which has been found in peripheral tissues, and even in the CNS, in some neurons, glial cells, and the pituitary gland. .
Physiologically, benzodiazepine receptors bind to a natural family of natural peptides called endozepines, which modulate various biological activities, either central or peripheral; synthetic molecules such as benzodiazepines mimic (at least partially) the effects of these peptides. Benzodiazepines, especially Alprazolam, have a clear inhibitory influence on the activity of the adrenal-hypothalamic-pituitary axis, effects that appear to be mediated through the hypothalamic and suprahypothalamic pathways.
The intense inhibitory influence of Alprazolam on these neurological pathways explains its particular efficacy in the treatment of clinical entities such as depression and panic. Alprazolam administered orally is rapidly absorbed, finding maximum plasmatic levels between the first and second hours after ingestion. Its bioavailability is 94%. Some high-fat foods two hours before the Alprazolam dose can increase its absorption. It is 80% protein bound and its volume of distribution differs significantly in young and elderly men, but not in women of different ages. The volume of distribution is greater in obese subjects compared to control subjects.
Alprazolam metabolism is carried out mainly in the liver via hydroxylation catalyzed by cytochrome P450, it produces 4 metabolites; alpha-hydroxyAlprazolam, which has a minimal activity of approximately half that of Alprazolam, although it does not contribute to its pharmacological effects, benzophenone, an inactive metabolite, desmethyl Alprazolam and 4-hydroxyAlprazolam which has less than a quarter of the activity Alprazolam, although it does not contribute to its pharmacological effects either. The elimination half-life of Alprazolam is 12 to 15 hours. It presents a renal elimination in 80%, which decreases in the elderly.
