Tramadol is a centrally acting opioid analgesic. It is a pure non-selective agonist at µ, d and k opioid receptors, with a higher affinity for the µ receptor.

Other mechanisms that contribute to its analgesic effect are inhibition of neuronal reuptake of norepinephrine and enhancement of serotonin release.

Tramadol has an antitussive effect. In contrast to morphine, in a wide range, the analgesic doses of Tramadol do not exert any respiratory depressant effect, and gastrointestinal motility is affected to a lesser extent. Its effects on the cardiovascular system are rather slight. The potency of Tramadol has been reported to be 1/10 (one tenth) - 1/6 (one sixth) that of morphine.
